Who can request a PSA death certificate?

Death certificates are confidential civil registry documents. They may be issued to:

1. The spouse of the deceased

2. The deceased’s children or parents

3. A person authorized via Special Power of Attorney (SPA)

4. A court or proper public official in official proceedings

Funeral service providers and insurance companies typically work through an authorized family member. Have a valid ID ready — the requester’s identity is checked at ordering and at courier release.

How to request online

PSAHelpline.ph (₱365/copy, delivered):

1. Go to psahelpline.ph → Order Now → Death Certificate.

2. Enter the deceased’s full name, date and place of death.

3. Enter requester details (name, relationship), delivery address, mobile, email.

4. Pay ₱365 per copy — cards, GCash, Maya, 7-Eleven, Bayad Center, BancNet.

5. Track with your reference number. Delivery: 3–4 working days Metro Manila, 4–7 working days provinces. Valid ID required upon receipt.

PSA Serbilis (₱330/copy): web form → print payment slip → pay at accredited banks → track online.

CRS outlet (₱155/copy): appointment mandatory since 23 March 2026 (book at appointment.psa.gov.ph) — bring a valid ID, pay at the cashier, same-day release possible when the record is available.

e-Certificate (₱290): digitally signed PDF — check whether the insurance company or agency accepts digital copies first; many still require SECPA paper.

  • Insurance claims: insurers usually want a PSA copy (not just the LCRO or hospital copy) plus the policy documents.

Troubleshooting

“No record found” for a recent death. Normal — the LCRO-to-PSA transmission takes weeks. Confirm registration with the LCRO first, then wait before reordering.

Wrong date or name on the issued certificate. Do not reorder — that is a correction case. Clerical errors go through RA 9048 at the LCRO; see the correction guide.

Courier will not release to me. Only the requester (or an authorized representative with a signed authorization letter and valid ID) can receive the document. If someone else ordered it for you, have them write the authorization letter.
