The Member Data Record is a one-page document generated from PhilHealth’s system showing:

  • Your 12-digit PhilHealth PIN and full name

  • Your membership status

How to download your MDR online

1. Log in to the PhilHealth Member Portal (create an account first with your PIN if you haven’t — see online registration).

2. Find the MDR / Member Data Record option on your dashboard.

3. Generate or download it — it’s instant, no waiting period.

4. Save a screenshot and a PDF copy on your phone. A screenshot is usually enough for hospital front desks; keep a printed copy in your documents folder as backup.

Keep it current: if you married, had a child, or want parents listed as dependents, update your MDR — benefit claims move faster when the names already match. Dependent additions need proof of relationship (marriage certificate, birth certificates).

Who counts as a qualified dependent?

At zero additional premium, coverage extends to:

  • Your legal spouse (not themselves an active member)
  • Unmarried, unemployed children under 21 — legitimate, illegitimate, legally adopted, or step-children
  • Foster children under RA 10165 (Foster Care Act)
  • Parents aged 60+ not otherwise enrolled under their own membership
  • Parents with permanent disability, at any age

Adding a dependent requires proof of relationship submitted with your PMRF or a dependent-addition form at a PhilHealth office.

How to get your first PhilHealth ID

1. Register and get your PIN — see online registration. The PIN is the prerequisite for everything.

2. Visit any PhilHealth branch, LHIO, or PhilHealth Express booth (in major malls) in person with:

  • Your 12-digit PhilHealth PIN
  • One valid government-issued ID (original + photocopy)
  • Your MDR (printed or on your phone) — speeds up verification

3. The card is issued on-site. Online registration only generates your PIN — never the physical card.

There are two card formats: the standard paper/cardboard ID (issued free) and the digitized PVC card (available at select branches with printing machines). Your standard PhilHealth ID is free — PhilHealth has reminded the public that ID cards are issued free of charge, so never pay a fixer for one. Some branches reportedly offer the digitized PVC format for around ₱90, but PhilHealth has not published a current official fee for it, so treat that figure as unconfirmed and ask your branch first. Both formats carry the same membership proof.

Tip: if you need proof of membership immediately (e.g., for a hospital admission tomorrow), the downloaded MDR is enough — don’t wait for the card. Your PIN never expires.

Lost PhilHealth ID: replacement steps

1. Some branches ask for a notarized Affidavit of Loss (state when and where the card was lost; notarization typically ₱100–₱300) — bring one along to be safe, but PhilHealth officially requires only the PMRF and two valid IDs below.

2. Accomplish a PMRF marked for replacement/updating.

3. Bring one to two valid government-issued IDs (originals + photocopies) and your PhilHealth PIN if known.

4. Submit at any PhilHealth branch or LHIO. The standard replacement card is free — PhilHealth has reminded the public the ID itself is free of charge (beware fixers charging extra). If you want the digitized PVC format, ask at the counter; a nominal fee (around ₱90) is sometimes reported but not officially published.

5. Damaged card? Surrender the damaged card instead of an Affidavit of Loss; the rest of the process is the same.

Troubleshooting

I can’t log in to the Member Portal. Use Create Account with your PIN if you’ve never set up portal access. If login fails, check that your registered email is correct and use the portal’s password-recovery option.

My MDR doesn’t show my dependents. They were likely never registered. Submit proof of relationship (marriage/birth certificates) at a PhilHealth office to add them.

Hospital says my PIN is “not found.” Your registration may never have been activated, or details don’t match. Verify your record on the Member Portal or visit the nearest LHIO with a valid ID.
