2. Accomplish a PMRF marked for replacement/updating.

3. Bring one to two valid government-issued IDs (originals + photocopies) and your PhilHealth PIN if known.

4. Submit at any PhilHealth branch or LHIO. A replacement fee of around ₱90 may apply for the PVC format — confirm on-site; the standard card is generally free, and PhilHealth has reminded the public the ID itself is free of charge (beware fixers charging extra).

Troubleshooting
