Requirements checklist

Step-by-step: online registration

1. Go to philhealth.gov.ph, open Online Services, and launch the Member Portal.

2. Choose PIN Application (for first-time registrants with no PIN).

3. Read and accept the terms and conditions to proceed.

4. Fill out the electronic PMRF: personal details, address, and membership category. Every detail must match your PSA birth certificate exactly — name mismatches (“Ma.” vs “Maria”) are the most common cause of rejection.

5. Upload your documents (PSA birth certificate or valid ID; proof of income if self-employed/voluntary).

6. Provide a working email address and complete the CAPTCHA.

7. Review every field, then submit. Watch for the acknowledgment email — check spam if it doesn’t arrive within a few minutes.

8. Wait for your 12-digit PIN to be sent to your registered email once registration is processed. That number is yours for life.

Alternative channels: the eGovPH app (Government Services → PhilHealth Portal) also supports registration steps, and you can walk in at any LHIO (Local Health Insurance Office) or PhilHealth Express outlet with two printed PMRF copies.

After you get your PIN

1. Create your Member Portal account (Create Account option) using your PIN — this unlocks contribution history, SPA generation, and MDR download.

Troubleshooting

Acknowledgment email never arrived. Check spam/junk first. If nothing after a few hours, your submission may not have gone through — try again in Chrome or Edge with clear document scans.

PIN hasn’t arrived after 5 working days. Contact the PhilHealth Action Center with your full name, date of birth, and submission date. Do not submit a second application — duplicates slow everything down.

Portal rejects my documents. Re-scan in good lighting, keep each file under the portal’s size limit, and make sure the name on every document matches your PSA birth certificate letter for letter.

I registered through HR but have no PIN. Ask your HR for the PIN or the PMRF acknowledgment. If HR never filed it, register yourself through the portal’s PIN Application.
